Evaluación de las lesiones tumorales de pie y tobillo

Resumen

El pie es una estructura anatómica terminal con compartimientos estrechos y con tejidos muy bien vascularizados, hecho que predispone a la diseminación metastásica hematógena o linfática; de hecho, y por regla general, la mayoría de los tumores malignos del pie y tobillo han sobrepasado, en el momento del diagnóstico, el compartimiento original. Adicionalmente, las manifestaciones clínicas de las lesiones tumorales del pie son con frecuencia inespecíficas, por lo que el diagnóstico suele ser tardío. Los objetivos del tratamiento comprenden el control local de la lesión, la restauración de la función y la supervivencia a largo plazo, lo que plantea en esta región anatómica condicionantes especiales. Se revisan en el presente artículo los condicionantes anatómicos que influyen en el diagnóstico y en la elección del abordaje terapéutico, para pasar luego a los procedimientos de evaluación clínica y estadiaje. Seguidamente, se examinan las características de las lesiones tumorales benignas y malignas habituales, considerando por separado las de partes blandas y las de origen óseo, comentando las peculiaridades de su tratamiento.

Abstract

The foot is a terminal anatomical structure with narrow compartments and highly vascularised tissus, a fact predisposing to haematogenous and lymphatic metastatic dissemination. In fact, and as a general rule, most of the malignant neoplasms of the foot and ankle have, at the time of diagnosis, exceeded the limits of the original compartment. Furthermore, the clinical manifestations of tumoural lesions in this region are quite often nonspecific, leading to a late diagnosis. The aims of therapy encompass local control of the lesion, restoration of function, and long-term survival; these aims pose, in this particular anatomical region, unique conditioning considerations. The present paper reviews the anatomic conditionants influencing both the diagnosis and the selection of the therapeutic approach, and then goes on to consider the diagnostic and staging procedures. Finally, the characteristics and features of the usual benign and malignant tumoural lesions are discussed with separate consideration of soft-tissue and bone neoplasms, and the peculiarities of the therapeutic management are commented.
