Luxación recidivante de hombro. Tratamiento con Bankart artroscópico. Estudio de las recidivas postquirúrgicas

Resumen

La cirugía artroscópica ha igualado en cuanto a resultados a la cirugía abierta mediante técnica de Bankart en la luxación recidivante de hombro, disminuyendo la morbilidad, tiempo quirúrgico y costes. Para analizar los factores que podrían influir en la luxación postoperatoria, entre 1997 y 2001, incluimos en un estudio prospectivo, 94 luxaciones recidivantes de hombro tratadas artroscópicamente y mediante técnica de Bankart. Mantuvimos constantes las variables: cirujano, técnica quirúrgica y rehabilitación, para evitar factores que alterasen el resultado. El seguimiento mínimo fue de 20 meses. Analizamos los resultados estadísticamente mediante la χ² y el Modified Rowe Score, y observamos la evolución de los diferentes pacientes en función de los factores anatómicos de la lesión y etiopatogénicos del paciente.

Abstract

As far as the results are concerned, arthroscopic surgery has equalled the open surgical management with the Bankart procedure in recurrent luxation of the shoulder, decreasing morbidity, surgical time and costs. In order to analyse the factors that might influence postoperatory luxation, we included from 1997 to 2001 in a prospective study 94 recurrent luxations of the shoulder managed either arthroscopically or with the Bankart technique. The variables “surgeon”, “surgical technique” and “rehabilitation” were maintained constant so as to obviate confounding factors. The minimum follow-up was twenty months. The results were analysed statistically using the χ² test and the Modified Rowe Score. The evolution of the patients was put in correlation to the anatomic factors and features of the lesion and the aetiopathogenetic factors in each patient.
